  5. Tampa General Hospital’s Dr. Peter Chang Discusses Quality Improvement through EHR Data

Recently Dr. Peter Chang, Chief Medical Informatics Officer at Tampa General Hospital, was featured in EHR Intelligence. Dr. Chang discussed the benefits of using LogicStream Health’s Clinical Process Measurement solutions for quality improvement using data from the electronic health record (EHR). Below we provide a few quotes from the article, but the full text of the feature can be found here.

Dr. Peter Chang, CMIO at Tampa General Hospital is focused on quality improvement

Dr. Chang also presented a webinar titled ‘Modernizing Quality Improvement through Clinical Process Measurement.’ In that presentation, Dr. Chang outlines how Tampa General Hospital has instituted a quality governance structure focused on improving clinical outcomes and physician satisfaction with the EHR.

    Quality Improvement Article quotes:

‘Using government incentive programs as a framework for modeling what clinicians should focus on when improving patient care, Tampa General is looking to get more out of its EHR data by laying technologies over the EHR system—in Tampa General’s case, LogicStream—capable of extracting information about care quality that can be made more useful and meaningful for clinicians.’

“The data LogicStream is accessing is straight from our Epic database,” noted Chang. “It’s a tool that sits on top of our Epic database that harvests basic things—what orders were enacted, how many medications were dispensed, how many drug-to-drug alerts fired, how many best practices advisories fired.”

“We are operating in a very reactive state now,” admitted Chang. “But we’re on the verge of using LogicStream to be a little more proactive in how we approach these quality measures. We’re chasing an improvement, but it’s all analyzing old data and trying to implement new processes, documentation, and workflow to help improve the outcome and drive patient safety.”

Combining EHR systems with technology from outside vendors capable of structuring large swaths of data accessible through health databases is already a big step in the right direction toward avoiding overwhelming providers with information. Still, Chang hopes for an even more streamlined future.

Patrick Yoder, CEO of LogicStream Health, recently published an article in the HITECH Answers blog. The article describes how health systems are benefiting from LogicStream’s Clinical Process Measurement solutions. Patrick Yoder, CEO, LogicStream Health

As health systems strive to become highly reliable organizations, they are focused on improving the quality of care they deliver to their patients. Difficulty often arises around standardizing and measuring the processes utilized by clinicians in care delivery and understanding how those processes impact clinical outcomes. Below are a couple excerpts from the article. You can read the entire article by following this link.

    From the HITECH Answers article:

By facilitating new ways to measure clinician workflows, Clinical Process Measurement (CPM) drives a deeper understanding of the impact those processes have on outcomes and how they propel highly reliable healthcare. Clinical Process Measurement helps improve patient outcomes and reduce costs by fully capitalizing on standardized clinical process. It accurately measures clinical care processes to provide a comprehensive understanding of the impact they have on patient care. Finally, CPM delivers a sustainable and scalable approach to quality improvement that historically has been out of reach for most healthcare organizations.

Below are examples of leading healthcare organizations realizing the significant advantages of deploying cloud-based CPM solutions:

    • Carilion Clinic applied LogicStream’s solution to monitor and measure the impact of clinical process on VTE and CAUTI rates so they could standardize processes to ensure optimal outcomes. The result was a reduction in care variation and reduced costs related to HACs.
    • Tampa General Hospital (TGH) utilizes the LogicStream CPM solutions to achieve quality governance that is scalable and repeatable. One of TGH’s first initiatives was to understand and improve sepsis bundle compliance. Clinical Process Measurement enabled identification of the patient population most at-risk for sepsis by pulling information from the EHR and analyzing it to find patterns and anomalies. It also monitors blood cultures, antibiotic administration, and fluids in the at-risk population to identify instances of non-compliance, all while measuring the compliance and effectiveness of the resulting educational efforts.

Tampa General’s Early Focus will be on Applying Clinical Process Measurement for CLABSI Reduction and Sepsis Compliance

LogicStream Health, Inc. announced today that Tampa General Hospital (TGH) will implement the LogicStream Platform to monitor and measure clinical process. Tampa General Hospital will first focus on reducing the rate of CLABSI and monitoring clinician compliance around TGH’s sepsis bundle. The hospital will also utilize the platform to accelerate clinical process redesign through enabling real-time assessments to achieve its vision of being ‘a leader of a clinically integrated network of providers serving West Central Florida.’

Tampa General is a private not-for-profit hospital and one of the most comprehensive medical facilities in West Central Florida, serving a dozen counties with a population in excess of 4 million. As one of the largest hospitals in Florida, Tampa General is licensed for 1,011 beds and, with approximately 7,300 employees, is one of the region’s largest employers. Tampa General Hospital has been affiliated with the USF Health Morsani College of Medicine since the school was created in the early 1970s. Tampa General is the primary teaching affiliate of USF Health.

To ensure its clinical delivery process provides the advanced care its patients require, TGH has assembled a governance steering committee led by Dr. Peter Chang, CMIO, and Dr. Laura Haubner, VP of Quality. The TGH governance team, along with key clinical representatives from LogicStream, will refine clinical process through optimization of clinical workflows, increased efficiencies and standardized clinical practice. The goals for a reduction in CLABSI and high compliance rate with TGH’s sepsis bundle will occur in the first six months of the three-year contract. To meet its aggressive timeframes, the team will leverage the LogicStream Platform to measure and improve critical clinical and non-clinical processes.

“With the LogicStream Platform, we will be able to measure our clinical processes prior to, during, and after implementation. This will allow us to identify and close gaps before they can negatively impact patient outcomes,” said Dr. Chang. “It is definitely a game changer for us.”

“The LogicStream Platform’s rapid assessment capabilities are key to TGH’s success and will give us early and leading indicators of what our outcomes will be six to nine months upstream,” said. Dr. Haubner. “The platform will also help us drive clinical standardization while optimizing our clinical decision support processes to achieve care quality objectives without disrupting clinical workflows.”

Unlike products that simply facilitate the reporting of health or quality outcomes, the LogicStream Platform supports strategic management and analysis of clinical care delivery and allows for clinical process measurement, something not offered by any other solution. Measuring the effectiveness of processes by assessing the clinical processes within the EHR, the platform can be utilized to improve outcomes, patient and provider satisfaction, and financial performance. The LogicStream Platform delivers specific solutions targeted at issues such as hospital acquired conditions, revenue cycle management, and regulatory compliance.

“Leading healthcare organizations like Tampa General know that sustained quality improvement requires insights that go deeper than anything available in or from today’s EHR solutions. A scalable approach that involves ongoing measurement of and enhancements to all clinical quality processes across the enterprise is required,” said Patrick Yoder, LogicStream co-founder and CEO. “Also, it must be available at the fingertips of clinical and quality personnel. LogicStream solutions provide clinical process measurement to positively affect patient outcomes while delivering insight into clinical care delivery that provider organizations don’t have today.”
